Application Process

Please follow the steps indicated below for applying at New Life Academy of Excellence. Please note that the Parent/Guardian of the student should submit the application.

  • Online applications will be available during open enrollment (December 15, 2023 through January 31, 2024).  Applications received after the Open Enrollment period will automatically be placed on a waiting list and you will be contacted upon space availability.
  • The application process does not guarantee enrollment.  We do not accept students on a first-come first-serve basis.  
  • Acceptance is solely based on the availability of seats in our program.

Lottery

If we have more applications than available seats the school will hold a public lottery for the upcoming school year. All qualifying applicants will be contacted of the date, time and place of the Lottery via email, phone and/or web-posting.        
      

Registration

The following steps should be completed after your child has been selected to attend NLAE. Please note that the parent or legal guardian of the student must be present at the time of registration.

  1. Once the student is accepted, parents/guardians will be receiving their acceptance letter and registration packet via mail. All necessary enrollment forms in the packet are to be completed and submitted on or before due dates given in your acceptance letter.
  2. Please complete and submit the following forms listed below along with your enrollment forms:
    • Certified copy of Birth Certificate.
    • Social Security Card or Waiver (Waiver must be notarized).
    • Form 3231: Immunization Certificate: Georgia requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophilus influenza, and varicella. Immunization information must be on the proper Georgia Department of Human Resources form; these are available through the Gwinnett County Department of Health and Wellness or local physicians.
    • Form 3300: Ear, Eye and Dental Screening: No child entering a Georgia school for the first time shall be admitted unless the child has a certificate of eye, ear and dental screening. This Georgia Department of Human Resources certificate must be signed by the Health Department or a licensed Georgia physician.
    • Form 3189: Additional immunizations for Sixth graders: Sixth grade students must have an additional MMR (mumps, measles and rubella) vaccine and are required to have the chicken pox (varicella) vaccine or proof of immunity. The Georgia DHR Form 3189 is acceptable for these students showing the additional immunizations for 6th graders. Forms 3231 and 3189 must be signed by the Health Department or a licensed Georgia physician.
    • Georgia Driver’s License.
    • Verification of County Residence (Any (2) forms listed below)
      • Mortgage Payment receipt or bill
      • Current Utility bill (gas, electric or water)
      • Lease Agreement-Signed by legal Guardian
      • Homeowner’s Insurance registration Card
      • Current Paycheck stub
      • Current Bank Statement
    • Proof of Guardianship (If Applicable)

Notification

Your child’s official enrollment status will be sent to you as soon as all applications have submitted by the deadline.  In order to guarantee your child’s placement, parents/guardians must return the signed enrollment form and present the required registration documents on or before the given due dates.
